Consent Form  to allow SSCD to use any images / information for SSCD Activities and documents

Self Disclosure Temporary CRB Statement of declaration (all members, including helpers and parents required to complete).  Full CRB checks are requested in line with policies and is processed at time of being a SSCD Member, including for all temporary members. Our Welfare Officer will contact you.
